Heart of a Champion Day is a free sports-specific health screening and educational program for high school student-athletes. The screenings include a review of the athlete’s medical history, a sports-specific medical and musculoskeletal exam, a heart exam, and a vision test.

The program is unique because of the heart screening, where all student-athletes receive an electrocardiogram (ECG), which traces the heart’s electrical activity. They also receive an echocardiogram, which is an ultrasound of the heart. A heart screening is necessary because traditional sports screenings may not reliably detect heart conditions that could affect an athlete’s ability to participate in sports.

Options for Payment of the Athletic Participation Fee
(August 2010)
 

I.  The preferred option is to pay online with a credit card or debit card.

• MasterCard, Visa, Discover cards accepted

• A 2.25% processing fee, on the total amount paid, will be assessed to your payment.

• You must have the student name and ID number to make an online payment.

• The online payment link is  https://webpay.easydraft.com/CS/CMS-Athletics. You can also find the online payment link on the CMS Website (click on Athletic Participation Fee).  

• Follow the prompt to either create a profile or to make a single payment. If you choose to create a profile, your information is stored for future visits to the site. If you choose to make a single payment, you will be required to enter your information every time to use the site.

• Follow the online prompts/instructions – note the drop down boxes and the required fields.

• Click on scholarship donations if you are making a donation to the general scholarship fund. All donations paid online go to the general scholarship fund.

• Make sure to print a copy of the Successful Payment page for your records.

• You will receive an email verification of your payment which you can email to the school athletic director as verification of payment.

• Contact your school athletic director with questions or concerns.
   

 II.      Pay with a check or money order directly to Myers Park High School (NO CASH).   

• Download the Athletic Fee Worksheet (Form).  Payment must be accompanied with the form filled out completely.  No payment can be turned in without this form as this information is vital to be able to properly credit the student athlete with the payment, which will keep them eligible.
   
• Make the check or money order payable to Myers Park High School.  
   
• You must have the student name and ID number to make a payment at the school.
   
• Payments can be made in the front office, marked to the attention of Kory Formon.  
   
• Coaches/athletic directors cannot accept payment of participation fees.
   
• Cash payments cannot be accepted. 
   
• Make sure to obtain and keep a receipt of your payment for your records.
   
•  Contact your school athletic director with questions or concerns.
